Does nighttime pain mean a torn rotator cuff?
Not by itself. The rotator cuff is a group of muscles and tendons that helps lift and rotate your arm. A tear can cause nighttime pain, particularly when lying on that shoulder, and weakness with lifting. But symptoms alone cannot confirm a tear, and some tears cause no pain.

What can you do while arranging care?
For mild symptoms without an injury or warning signs, keep the shoulder gently moving within a comfortable range. Reduce activities that clearly aggravate it rather than repeatedly testing a painful lift. Avoid starting strenuous shoulder exercises or heavy lifting to work through the pain.
These are general starting points, not a rehabilitation prescription. The NHS shoulder-pain guide advises getting assessed if symptoms worsen or have not begun improving after about two weeks. Marked difficulty moving the arm warrants earlier attention.
When should you seek urgent help?
Get prompt medical care for sudden severe pain, inability to move the arm, a change in shoulder shape, significant swelling, persistent numbness, or fever with shoulder pain. Pain after a fall or accident also needs timely assessment.

What information helps at your first visit?
You do not need to arrive with a diagnosis. A few observations can make the conversation more useful:
- When did it start, and was there an injury or change in activity?
- Does it hurt only when lying on the shoulder, or in other positions too?
- Which everyday tasks are difficult?
- Have you noticed weakness, numbness, or symptoms extending down the arm?
- What have you tried, and what changed?
For example, “I can lift groceries, but reaching overhead hurts and I wake up when I roll right” tells a clearer story than “my shoulder is bad.” Bring any existing imaging reports and a list of questions.
